I have found what film I was referring to and thankfully it wasn't some Croatian snuff film!
The title is called Lehrjahre eines Teenagers.

Thanks for the reply JJ. Yes it is that part of the film that obviously stood out, and it was literally that part that was on when I initially pressed play all those years ago. The rest transpired as I described. Just as an aside the insertion wasn't perverse or violent and appeared to be done ...
