A simple clear plain English release should work for every jurisdiction, unless a jurisdiction requires specific forms of words etc, (in which case you will need multiple forms)...
The more detailed and complex you make it, the less likely it is to be valid in the UK at least, because the courts here make it a rule that people should reasonably have expected to understand the implications of anything they sign.
They sign a simple one page "I am in a porn film and it can be used anywhere" and they have no get out. They sign 10 pages of legalese and stand up in court and say "but they SAID...." and you are stuffed (at least re inexperienced models).

I use a standard model release form, along with a 2257 form... that does however mean for 9 pages for the model to work through... but it covers all the bases
