How exactly is the 4th official meant to know who's meant to be coming off?

Not having seen the whole incident and nobody saying exactly how long there were 16 on the pitch its hard to point the finger at the 4th official
but more so at the haphazard way interchanges are made

As I said all interchanges like in soccer should take place on the sideline
1 goes off 1 comes on-seems simple-should be a walk in the park considering all those tertiary educated players on the park

So who was the braindead Pom who FORGOT to come off when his replacement arrived?

Did Luger get a cap for his appearance??. Anyhow I think Dan Luger is Slav extraction, Croatian I believe but I may be wrong.
