I would imagine a2m without proper hygiene will result in the "sucker" receiving a very nasty dose of Campylobacter food poisoning, followed by 3 days on the toliet.

I do recall an interview with Ashlyn Gere who said a shower, an enema and an applicaion of some antiseptic lubricant were all required before an uphill skiing scene took place. Also plenty of mouthwash, before and after.
