Rude bits accidently get to air.

I remember seeing a doco about 80's porn star Jeanette Starrian ( or Starrion ? ) on SBS - which is roughly the Aussie equivalent of Britains' Channel Four I think. They can show some pretty rude stuff without any complaints. Anyway, this show had the usual stuff you'd expect like tits and arse and a bit of pubes - fairly normal for SBS.
But then near the end they showed some shots she did with her husband Ray Victory who was also in porn. It showed for at least 4 or 5 seconds close-up shots of Jeanette sucking Rays' dick and then lucky Ray with his knob up her twat. None of this pixelation crap or soft focus. I was amazed and expected to hear that it was a mistake, but not a mention of it was made.
Amazingly, it was repeated again about a year later with those scenes intact.
I'm curious to know if similar things have been broadcast in the UK, either by mistake or deliberate and if anyone made a fuss.
And two flys fucking on the set of Neighbours doesn't count !
