I think the Amanda Charles site has a couple and also I got one from HGP Vidoes (both of which you can find via the Links.)
Not sure if the HGP one is listed on the website, I think I ordered it from a catalogue they sent me.

Well, my partner in crime has indicated that he does have a few bob to spend on this particular endeavour and to that end and in-between DVD authoring, I did rummage though a box of old VA movies yesterday evening. Furthermore, we expect a great leap forward in progress this week and consequently I have asked my printer to quote for the job this very afternoon. So, the great day does draw inexorably closer.
