adult films, Plot: Hospital. Or Hustlers

Hello,
I'm looking for hardcore films acting in hospitals or surgeries. But there should be rather a realistic story. Most of the films I know are stupid and unrealistic, only bad actors and actresses acting in disguise!
Another plot I'm looking for is s.th. about prostitution. This also should be realistic. Preferably with a real film story, or reportages, hardcore but s.th. corresponding to reality!
Has anybody any suggestions?
