(1) Do male performers "screw up" sometimes when it comes to the "money shot"?
Sometimes they cum too soon, and sometimes they can't cum at all.
When they fuck up a scene too often, they don't get "invited" back, and word quickly
gets around that the actor is not "dependable" and his porn career is cut short.
This explains why you keep seeing the same 10-20-30 or so actors in just about every movie
that gets released during a certain period of time.
Being "dependable" means you cum on cue when the director decides it is time
to deliver the goods.
When they can't cum, usually the director has to call in a "double" if he does not
plan to pay and reshoot the scene with another female-male.
And if you look closely sometimes (for old and occasionally even new scenes)
some "cum shots" are faked. How can you tell? When you don't see the sperm
being expelled from their dicks, when the girl keeps the dick in her mouth (the male

Well, porn filmmaking is not designed for males with (premature) ejaculation problems.
If you are familiar with BTS (behind the scenes) scenes often found in DVDs
you will notice how many breaks are taken during the production of a sex
scene. Generally it may take a couple of hours to film a porn scene.
Several breaks occur and are taken by the director and performers: to drink, bathroom break,
reapply make-up, snap some publicity stills, change set-ups/locations, etc
When all is said and done, the scene is edited down to 25-35-45 minutes
and released for a web site or DVD.
Making a porn scene in "one single long take" is usually in the realm of amateur (home video) productions

Reflecting back on this question, if you go back to some older scenes with Lexington Steele
whether it was part of an act or quite likely real, he seemed to be quite into the scene and into
his female partner. Several times you see him pull out (being supposedly on the brink of
"cumming" and walk away for a few seconds/minutes to compose himself and begin fucking again.
Perhaps Nacho Vidal may have done that in some of his scenes, but
I really don't recall other male performers behaving the same way on screen (or
whether the director allowed that in the final edit).

Well that is generally established at the outset, when the girl is booked for the
shoot (the male she will work with and what sex performance she is
expected to do, based on the pay scale in question).
It is not the girl's problem whether the male partner in the scene cums too soon or not at all.
That responsibility falls on the male. Actually you have to praise the girl who occasionally
has to "cover" for the male's "deficienies" (wood problems, tiny or inexistent
ejaculations, etc). When the guy is jerking off (for several minutes) and can't seem
to cum, and when he finally does so, there is nothing in the well, the girl does her best
with spittle to make it appear his load was substantial.

There are male performers who occasionally do no abide by the set rules and go rogue during a
shoot, at the dipleasure of the female performer. Mr Marcus was said to perform
vaginal/anal creampies even though the girl had strictly forbidden such conduct.
