Well, I am not a lawyer, but I am happy to say that I would be more than satisfied using that release myself, (As I indeed do) under pretty much any 'normal' circumstances.
Dont forget, you also need two copies of the PHOTO IDs provided by the model, along with a photo of her holding them up either side of her face.
You must also check that the details written on the form MATCH those on the IDs, and take at least a cursory glance to check the IDs are genuine.
Get the form witnessed, by someone if possible.
Also copies of certs are a good idea as well...
I'm sure someone else will give their opinion, and theres no reason why they may have a different opinion, however all I am saying is that I personally am happy using that release - However, DONT just take my word for it - A stranger on a forum - Consult a legal professional if in any doubt.
PS: Im not suggesting that models will bring fake IDs to a shoot or anything like that. I am simply talking from the point of view of you being able to truthfully say, you took all reasonable professional precautions, when it came to your paperwork
