I haven't had a flutter for many years, but am considering having a few quid on the space shuttle burning up during re-entry.
Does one have to find a bookmaker who is already offering odds on such an eventuality, or does one simply ask any bookmaker for a quote?.

There are lots of different speciality bets with many bookmakers and that would fall into that category.
You may have to ask and you may not. Check out there sites.
However many bookmakers have a rule that prevents them paying out in "acts of god" and that may come into that category.
I do think you are a bit morbid in betting on people to lose their lives though.
