I used to used Strongbox but it kept killing my servers for some reason which we couldn't work out why if i had a large dictionary attack.
The servers were beefy dedicated dual xeons which was more alarming although saying this, many people swear by SB.

As Joe said, Ray is a great guy but a little hard to get hold off now and no longer uses ICQ.

is a great alternative and pretty cheap at $49 a month per server.
