Well, I am not a lawyer, but I am happy to say that I would be more than satisfied using that release myself, (As I indeed do) under pretty much any 'normal' circumstances.

Dont forget, you also need two copies of the PHOTO IDs provided by the model, along with a photo of her holding them up either side of her face.

You must also check that the details written on the form MATCH those on the IDs, and take at least a cursory glance to check the IDs are genuine.

Get the form witnessed, by someone if possible.

Also copies of certs are a good idea as well...

I'm sure someone else will give their opinion, and theres no reason why they may have a different opinion, however all I am saying is that I personally am happy using that release - However, DONT just take my word for it - A stranger on a forum - Consult a legal professional if in any doubt.

PS: Im not suggesting that models will bring fake IDs to a shoot or anything like that. I am simply talking from the point of view of you being able to truthfully say, you took all reasonable professional precautions, when it came to your paperwork :)

Yes, sorry. They ARE supposed to be government issued IDs. I forgot to say that, cause I wanted to get as much stuff down, off the top of my head, but not come across as a know it all.

I usually shoot 'known name models' - as opposed to 1st timers, so the issue of a model NOT having 2 Gov Issued IDs, has not been a problem.

Thing is, I dont know it all. I just know what applies to ME.

Which is why I also suggest that the OP should consult a legal expert, if he is unsure.

That said though, I think I covered most things, but if anyone else wants to add... Feel free :)

Hey snow.

Great model release, I might start using it as it is better than the one I already have. Thanks for that :)

As for copies of ID's, do you get the models to photocopy them and bring them along?

Get the models to bring the actual iDs and take a high quality digital photo of the IDs. They also need the IDs to hold up by their faces.
