Middle lane drivers don't bother me at all. If someone hogs the middle lane I'd undertake/overtake him/her. Sometimes undertaking is the only way to get by - it may technically be illegal, but what can you do?
The only thing that bothers me is people who don't know which lane to use on a roundabout - and don't indicate when coming off, leaving everyone to guess where they're going.
